Social Work

Post-MVA, a social worker delivers services that are crucial to the individuals’ rehabilitation within a multidisciplinary team. Their role is to collaborate with other professionals, communities, and organizations to provide services, improve conditions, and create opportunities for growth, recovery, and personal development. The social worker, while using a holistic approach, focuses on improving the individuals’ health and social well-being by evaluating the social determinants of health framework when providing care, navigating systems, and advocating for accessible improvement of the multiple treatments that impact the individuals’ health and well-being.

Depending on the severity of the individuals’ physical and psychological symptoms, a social worker may assist in the following ways (but is not limited to):

  • Conduct intake assessments, develop intervention/treatment plans pertinent to the individuals’ situation.
  • Assist the individual in navigating available social and community resources and supports.
  • Conduct appropriate referrals as needed fitted to the individuals fluctuating needs.
  • Assist the client with completion of required documentation and applications.
  • Provide crisis intervention.
  • Provide ongoing social and emotional support to individuals and their families as needed.
  • Monitor third-party administered therapy.
  • Consult with other healthcare professionals involved in treatment to ensure the rehabilitation process is running smoothly and that the clients’ needs are addressed in an appropriate manor.
